1. Fill out the application completely and sign.
2. Attach at least two of the following pieces of information: 1 month of pay-stubs, letter from employer, recent tax return (form 1040 or 1099) or any income/employment verification.
3. Make payment of Fifty Dollars ($50) payable to “Housing Network Management, Corp”. This is a fee per credit check, which is non-refundable. This payment can be made in cash.
4. The application can be delivered in either of the ways described below:
   1. Bring the completed application with the $50 (credit check) to our office located at, 275 Park Avenue, Brooklyn NY 11205, (718) 237-2522.
   2. Or mail the completed application with the check (do not mail cash) for $50 (credit check) to our office located at, 275 Park Avenue, Brooklyn NY 11205, (718) 237-2522.
5. If you are approved then we require two checks (no personal checks), one (1) month security and one (1) month rent upon signing the lease for the apartment both made out to:
   THE CHOCOLATE FACTORY LLC
6. Keys will be given only after checks are received, lease is signed and on the beginning date of your lease unless other arrangements are agreed on. Security keys for the building entrance require a $10 security deposit each.
7. If you are a primary contributor to the rent or want to be a signer on the lease you must have your credit checked and income verified.
